By receiving services via telehealth, you acknowledge and agree that:
- Telehealth involves electronic transmission of your health information and may include video, audio, and/or data communications.
- The potential benefits include improved access to care; the potential risks include rare technology failures or interruptions.
- You may withdraw consent and request in-person alternatives (where available) at any time without affecting your right to future care.
- You will confirm your physical location at the start of each session. Services are provided only where your clinician is licensed and you are physically present.
